How to get a Modoc County Library card
- A Modoc County Library card is issued free of charge to California residents. Out-of-state residents will be charged a $20.00 annual fee.
- The Modoc County Library Card Application requires the following information:
- Name, phone number, and current mailing address
- Date of birth
- Driver's license or California ID (shown as photo ID)
- Name and phone number of a Modoc County reference
- Name, phone number, and current mailing address
- The application card of a minor must be signed by the parent or legal guardian who has a Modoc County Library card in good standing.
- A patron is financially responsible for all materials checked out on library card. The parent or legal guardian is responsible for materials checked out on a minor's card.
- A library card is valid for 2 years (1 year for out-of-state residents) unless revoked. A library card in good standing may be renewed.
How to replace a lost library card
- There is a $1.00 fee to replace a lost library card.
