How to get a Modoc County Library card

  • A Modoc County Library card is issued free of charge to California residents.  Out-of-state residents will be charged a $20.00 annual fee.
  • The Modoc County Library Card Application requires the following information:
    Name, phone number, and current mailing address
    Date of birth
    Driver's license or California ID (shown as photo ID)
    Name and phone number of a Modoc County reference
  • The application card of a minor must be signed by the parent or legal guardian who has a Modoc County Library card in good standing.
  • A patron is financially responsible for all materials checked out on library card. The parent or legal guardian is responsible for materials checked out on a minor's card.
  • A library card is valid for 2 years (1 year for out-of-state residents) unless revoked. A library card in good standing may be renewed.

How to replace a lost library card

  • There is a $1.00 fee to replace a lost library card.
